Where: PROVENCAL SCHOOL

What: Bring the following forms to register. Your child DOES NOT need to attend.

Please bring the following documents:

Copy of child’s most recent immunization/shot record
Copy of your child's birth certificate; Child must be 4 years old BY September 30th
Copy of your child's social security card
Your license or another type of photo I.D.
One of the following: Most recent utility bill, water bill, mortgage agreement, or rental agreement

One of the following - if applicable:

2 most current check stubs for EACH parent/guardian living in the home or a letter from your employer stating your rate of pay and hours included. If your child receives SNAP and/or Medicaid, there is a place to select this on the online registration so no documentation needed.
Any court papers pertaining to your child - if applicable

Your child’s application to preschool will not be complete until all documents are uploaded and approved. Classroom spots will be filled in the order that the registration is complete. Acceptance letters will be mailed out in June to those children who have earned a spot.
